Sanford Health Receives Major Grant
SIOUX FALLS, SD -

Sanford Health has been given yet another major money gift, only this one has nothing to do with local philanthropy.

The $12 million grant from the Centers for Medicare and Medicaid is the largest ever awarded to Sanford.

The grants are given to applicants determined to be able to quickly implement new ideas for ways to deliver better health care and lower costs.

Sanford plans to use the money to expand its Medical Home Program, which is designed to keep people with chronic health conditions in their own homes for longer periods of time.
