Adams Thermal Systems in Canton will pay more than $1.3 million in fines after death at the plant in 2011.
The accident occurred when an OSHA investigation found that a worker had been fatally crushed in a machine used to make radiator core. The employee was instructed and authorized by management to bypass the manufacturer’s barrier guard in order to adjust the machine to keep it running.
Shortly after the accident, OSHA conducted two concurrent safety and health investigations and found 66 different violations.
Under the agreement the worker's spouse will get $450,000.
